By submitting the form above you hereby agree that Universal 1 Credit Union, Inc., and those authorized by Universal 1 Credit Union, may use, prepare, manipulate, and reproduce images and/or audio statements taken of me in digital, video, photographic and/or other audio/visual format. Such images may be distributed, publicly displayed, and presented for commercial or educational purposes through any media now known or later developed, including, but not limited to: Web sites, social media, print media, television, or radio for use in conferences, seminars, magazines, newsletters, publications, newspapers, billboard advertising, event materials, and/or other collateral marketing materials, without future compensation to me with respect to a member testimonial project. I understand and agree that my first name and city of residence (if completed below) may also be published.
I waive the right to inspect or approve the finished work or its application. I agree that recordings in any format and reproductions belong solely to Universal 1 Credit Union, to copyright under its own name and with full right of disposition in any manner whatsoever.
I have read this authorization and release and I am familiar with its contents. This release shall be binding upon me, my heirs, legal representatives, and assigns.
